You should receive an email confirmation with your payment receipt and a copy of the information entered on your registration form. If you do not receive the email, please contact us for confirmation and a receipt.

We must have two medical forms on file for every SummerSault camper, linked below. 
Physical Exam Form​ 

• Please print out this form, and have your child's doctor complete it.
  
• Mail completed forms to: SummerSault, 540 East 76th Street, New York, NY 10021. 

Camper Health Information Form 

• You can fill out and submit this form online. 

Billing: If you did not choose to pay your full tuition with registration, we will send an invoice for the remainder of your tuition, due within two weeks of receiving your bill.

• Please note: If the remaining tuition balance is not received within two weeks of receiving your bill, we will not be able to hold the camper's spot. Thank you in advance for timely payment!
