- A registration fee of $175 ($100 before April 12!) and a $500 deposit are due with the form, payable by credit card.
- Mail us your child's health forms (download links below) as soon as possible.
- Mail a check for the balance of your tuition (we will send an invoice) within two weeks of receiving your bill to ensure your child's spot.

Required Health Forms
We must have two medical forms on file for every SummerSault camper at Trinity School. Please check your confirmation email for links to the forms. Please mail your completed forms to SummerSault at 540 East 76th Street, NYC 10021.
- Physical Exam Form (to be completed by your child's doctor)
- Health Information Form
Tuition Payment
We will calculate your tuition based on your registration form and send a bill to the camper's home address.
